CONSULAR OUTREACH PROGRAM TO MONTREAL

SPECIAL INSTRUCTIONS FOR PASSPORT APPLICANTS
The Application Form
Read application form carefully.
Do not leave any blanks. If question is not applicable to you, write N.A.(Not Applicable).
Prepare 3 recent colored photographs with royal blue or dark blue background (e.g. blue in Quebec coat of arms).
For the birth data (place of birth), specify the town/city and province.
Be sure to fill-out foreign address completely and correctly. Your new Machine-Readable Passport (MRP) will be mailed to this address, and it may be lost if address is incomplete or incorrect.
Note*: (For passport renewal applicants who lost their old Philippine passports, an affidavit of loss, police report and copy of the lost passport should be submitted together with the duly-accomplished application forms and appropriate fees in postal money order).
Mailing the Application Form
Mail duly-filled out application form together with: (A) three passport-sized pictures (one pasted and two enclosed inside a small transparent plastic pouch with zip lock) on royal blue background (two ears visible, no hair should touch the eyebrows, attire should be with collar and sleeves, no eyeglasses, for males- there should be no earrings); (B) original old green passport to be renewed; (C) all supporting documents [ (1) Xerox copy of old Philippine passport (1st page only); (2) Xerox copy of work permit or permanent resident card (front & back)]; (3) affidavit of citizenship; if applicable (if you have been a permanent resident for 3 years or more, you have to execute an affidavit of citizenship stating that although you are already privileged to apply for Canadian citizenship, you still don’t want to or if you have already applied, it is still under process); (D) xpresspost return envelope with signature sticker and with complete and correct return address and name for the safe delivery of both the old (if there is any) and the new passport; (E) a postal money order consular fee cheque in the amount of C$57.50 for the renewal of passport; and, (F) C$28.75 for the affidavit of citizenship (if applicable) payable to the Philippine Embassy.
(Note*: For lost passport applications, a postal money order cheque in the amount of C$103.50 (if the lost passport was issued in Ottawa) or C$129.50 (if the lost passport was not issued in Ottawa) payable to the Philippine Embassy, must be mailed together with the required documents); All passport applications should be sent to: the Philippine Embassy at 130 Albert Street, Suite 606, Ottawa, Ontario K1P 5G4, Canada.
The Embassy must receive your MRP application on or before Friday, 18 September 2009, so that all the required data and information you supplied on the passport application form can be encoded onto the data-capturing device of the Embassy thus making it possible for the computer-generated forms (CGFs) to be printed at the Embassy and brought to Montreal on Saturday, 26 September 2009.
Personal Appearance on 26 September 2009
You must personally appear during the Outreach Program on Saturday, 26 September 2009, to personally affix your left and right thumb prints and appropriate signature on the CGF;
Your personal appearance on Saturday, 26 September 2009, completes the MRP application process. The applications will be sent via private courier service to the Department of Foreign Affairs (DFA) in Manila on 02 October 2009 (the private courier’s scheduled service). The finished passports will then be mailed to your indicated address at least 30 working days after 02 October 2009.
ONLY APPLICANTS WHOSE APPLICATIONS WERE RECEIVED BY THE EMBASSY BY 18 SEPTEMBEER 2009 AND WHO STRICTLY COMPLIED WITH THE ABOVE PROCEDURES WILL BE ABLE TO COMPLETE THE MRP APPLICATION PROCESS DURING THE OUTREACH PROGRAM.
ALL OTHERS, INCLUDING WALK-IN APPLICANTS FOR PASSPORT SERVICES, WILL STILL HAVE TO GO TO THE EMBASSY IN OTTAWA TO COMPLETE THE MRP APPLICATION PROCESS.
OTHER SERVICES
Full notarial services (jurat, affidavits, special power of attorneys, and certifications) and consular services (balikbayan stamps, fingerprint impressions for NBI clearance, passport amendment from maiden to married name).
Other services such as visa applications, reports of marriages and births, dual citizenship applications are always processed in Ottawa, so only applicants who strictly comply with the complete requirements may be initially accommodated and accepted but the processing will only be done in Ottawa later.
